Background Project MyPhIS

♦ MyPhIS is an acronym for Pharmacy Information System
♦ Pharmaniaga Logistics Sdn. Bhd. (PLSB) was awarded a 10 years contract since March 2011 and subsequently renewed until June 2030 to supply products and system to all hospitals and clinics under Ministry of Health.
♦ The system was piloted at six Hospitals, one District Health Office and 13 Health Clinics in 2013/ 2014. Expansion activities were carried out since 2014 to 1,342 health facilities under Ministry Of Health throughout the country.

Privacy Policy
The MyPhIS Application (hereinafter referred to as the "Application"), is an integrated pharmacy information system developed to support the operations and management of pharmacies at MOH health facilities such as Hospitals, Health Clinics, and District Health Offices (hereinafter referred to as "health facilities"). It is part of the national digital health initiative to increase efficiency and accuracy in the delivery of pharmacy services.
MyPhIS is owned and operated by the Government of Malaysia. The Application is administered by the Pharmaceutical Services Programme, Ministry of Health Malaysia (MOH). The Government ensures that the personal information collected is in line with the provisions of the Personal Data Protection Act 2010 (Act 709).
Personal Information
This Application will record the Personal Data of patients who have registered at health facilities for the purpose of seeking treatment. The information submitted is used solely for the purpose of pharmacy services at the health facilities. This information will not be shared with any other organization for purposes other than those specifically stated.
Information Collected
This Application will only collect data required for healthcare purposes through the registration process at health facilities. The data is used to enable the dispensing, verification, and monitoring of medication receipt through the Application.
Data is obtained manually during the patient registration process at health facilities, and subsequently through prescription information as well as records of medication administration to the patient.
The Personal Data typically collected is as follows:
- Patient name
- Identification card number/Passport
- Phone number
- Date of birth
- Name of health facility
- MRN (medical record number)
- Home address
- Future doctor's appointment date
- Email address
- Medication information prescribed by a medical officer
- Medication dispensing information
- Patient clinical information
Patient medication information:
- Preparation and supply of medications to patients
- Communicating with patients for information verification
- Providing feedback to patients
Data Collection Purposed
Personal data collected will not be used for any other purpose and will not be disclosed to third parties or transferred outside of Malaysia for commercial purposes.
Consequences If Personal Data Is Not Allowed
If the Personal Data provided is insufficient or unsatisfactory, the process of supplying medication to the patient will be affected.
Confidentiality
Personal Data will be kept confidential in accordance with the laws in force.
Data Security
This Application implements security features that comply with global standards. Data in transit is encrypted and data at rest is stored with a high level of security.
